Emergency Care Planning


Carers Northumberland can support you to make an Emergency Care Plan to document important information about the person you care for. 




Why plan for an emergency?

By planning in advance for emergencies you can have peace of mind that the person you care for is looked after, you can ensure a speedy response if you’re taken ill, you can have people on standby so they are not left alone and know that all of their important information is in one easily accessible document. 

How does it work?

Carers Northumberland will support you to complete a template that has all of the important information about the person you care for.  You can then have as many copies of the document as you like to give out or carry yourself to ensure peace of mind.


You can also choose just to be given the blank template to complete yourself.

Other things you can do

  • Talk to family, your friends and your Care Manager
  • Use the “Message in a Bottle”; A plastic bottle in your fridge holds your important details
  • ICE - put ICE (In Case of Emergency) in your mobile phone contacts, with the number of the person to be contacted in an emergency.
  • Arrange lasting power of attorney using Carers Northumberland's Advance Planning Service
  • Get a carers assessment to put additional support in place for you and the person you care for
